********Dance
Performance Application for Accepted Groups / Couples********
The
Groups/Couples making a Dance Presentation for the
2012 7th Annual Children Teen Salsa Congress agree
to the following procedures

| 1 |
Fill
out an application form so groups/couples can perform. |
| 2 |
Send
the list of names of each performer in your group/couple
on the application form. |
| 3 |
The maximum time for each group presentation
will be five (5) minutes. For couples the maximum
time allotted for presentation is three (3) minutes.
Only We reserve the right to approve
requests to extend the time of the dance presentation.

| 4 |
We
Reserve the right to assign the day or night of the performance
for each group/couple. |
| 5 |
Music
must be provided on a compact disk. Please
have just one number on the CD. |
| 6 |
All
performers will receive a complimentary 3-day Evening
only congress pass for the weekend.
There are no workshops at this congress. |
| 7 |
Dance applications should be in our
office no later than June 20, 2012. |
| 8 |
The
congress will have a "Hospitality Desk" for
all dancers. Upon check-in and registration for the congress,
all performers will receive a schedule of performances. |
| 9 |
We reserve
the right to make changes concerning the date, time
and place of each presentation.
